In the year 1998 Dr. Nagaratna along with a group of like-minded friends started ASM Trust in Bellary with a single focus of -all round benefits of elders and addressing all the basic problems of Senior Citizens.

'Senior citizen cell and helpline' with Superintendent of Police, District Collector and group of judicial experts in the panel - is providing 24 X 7 emergency services to unprotected senior citizens staying in their residences.

'Home Again' (home for the aged) was launched in 1998 and over 1000 senior citizens have been served either as inmates or through home care services.

In the last few years, the management of these services has faced an increasing number of queries regarding housing options for seniors. The concept of housing for seniors, frequently referred to as old age home, in India has, in the past, been thwarted by a social stigma. There is a change in the last few years which has witnessed a marked increase in nuclear families and an increasing trend of children of ageing parents moving to other countries, ultimately leading to families no longer being equipped to take care of their ageing members and also an increasing number of seniors who live alone and to some extent struggle with running a household. 'Krishna Sannidhi' a stay home for the aged, was launched in 2012, as a home for seniors who prefer to avoid the hassles of maintaining a household and some times are parents of NRIs .

Over the years the establishment of 'Home Again' brought to the founder's notice a segment of seniors who needed medical and non-medical attention (which their family caregivers where unable to fully provide) but who did not wish to leave their homes or to move to a nursing/medical facility.

ASM-VTP bed side assistants and home nursing care institute was started in 2012 and students who have learnt in this institute have been working in Dr. Nagaratna long term care hospital assisted living centre and are being sent to residences of elders who need their services.
